PHP File Upload from Form

Hey, All!
I'm generally pretty good at PHP, but I have not yet had to create a form that uploads a file from the user's computer.
As there does not seem to be a ton of "how to" information online on this subject, is there anyone who can help me out with how to write up the proper form tags and receiving PHP code?
