Use a professionally signed certificate, which fields are required?

I've read the wiki about buying a professionally signed certificate.
The form requires many fields, but there are no *asterix saying which ones are required:
1. First Name
2. Last Name
3. Email Address
4. Phone Number
5. Organization Name
6. Name of your Security Department
7. Address
8. City
9. State
10. Zip
11. Country
A) Which ones are required?
B) Which ones show up in the browser's SSL Server Certificate panel "Issued To" and does that vary from browser to browser?
