vBulletin Configuration Assistance

I've been having issues getting vBulletin to work on my new hosting. It's issues with the configuration and it asks me for:
- Database Type
- Database Name
- Table Prefix
- Server Name + Port
- Master User Name/Password
Now, in all truth I seriously think I filled out all of that information correctly. But, for some odd reason, it doesn't seem to be working and it gives me a database error. Explain to me exactly what I should fill in for all of the above things I've posted please. And also, is it necessary to lay it out like "accountname_databasename" and so forth?
